Referrals & Care Coordination

 


Isabelle will do a brief (non-clinical) Care Assessment of you or your loved one. She will then inform you of appropriate services or providers, including social workers and nurses who specialize in geriatric care management and home health aide agencies.  She can also coordinate the care (interviewing, scheduling, managing, unannounced drop-in checks) you or your loved one is receiving. [Note that there is NO reward of any kind from any provider or company Isabelle refers you to. She has high ethical standards.]

 

Appointment Scheduling and Transportation (no wheelchairs)

 

Isabelle is happy to help with scheduling medical and other appointments, and may be able to drive you or your relative to and from facilities and offices (as long as mobility limitations do not exceed a foldable walker or a cane). She can also wait on-site or accompany you to the provider, take medical notes, pick up medications, etc. (Note that these are billable hours.)

 
 

Logistical and Emotional Support

 

Isabelle is excellent at logistics and can map out the most efficient schedules, coordinate timing of appointments, etc. She is also deeply compassionate and caring, comfortable with people of all ages and backgrounds, has experience with many folks who suffer from chronic conditions, including seniors in various stages of dementia. Isabelle also provides emotional support of (and communication with) the concerned family members.

 

Friendly Visits and Cultural Outings

 



Isabelle is bilingual in French, enjoys music, theater, movies, board games, playing cards, listening to people’s stories.  She considers it an honor to visit you or your loved one, happy to help with visiting museums, concerts or other cultural events whether in person or online, or just take a walk together and enjoy a cup of coffee or tea.
